Right Ventricular–Pulmonary Arterial Coupling in Secondary Tricuspid Regurgitation

چکیده
Chronic pressure-overload induces right ventricular (RV) adaptation to maintain RV-pulmonary arterial (PA) coupling. RV remodeling is frequently associated with secondary tricuspid regurgitation (TR) which may accelerate uncoupling. Our aim determine whether the non-invasive analysis of RV-PA coupling could improve risk stratification in patients TR. A total 1,149 (median age 72[IQR, 63 79] years, 51% men) moderate or severe TR were included. was estimated using ratio between two standard echocardiographic measurements: annular plane systolic excursion (TAPSE) and pulmonary artery pressure (PASP). The all-cause mortality across different values TAPSE/PASP analyzed a spline analysis. cut-off value identify uncoupling based on curve At time significant diagnosis median 0.35 (IQR, 0.25 0.49) mm/mm Hg. 470 (41%) demonstrated (<0.31 Hg). Patients presented more heart failure symptoms had larger left dimensions, compared those During follow-up 51 17 86) months, 586 (51%) died. cumulative 5-year survival rate lower their counterparts (37% vs 64%, p < 0.001). After correcting for potential confounders, only parameter independently (HR 1.462; 95% CI 1.192 1.793; In conclusion, poor prognosis stratification.

Secondary tricuspid valve regurgitation: a forgotten entity
To cite: Tornos Mas P, Rodríguez-Palomares JF, Antunes MJ. Heart 2015;101:1840–1848. INTRODUCTION The tricuspid valve was virtually ignored for a long time in the past. However, the incidence of tricuspid insufficiency associated with left valvular disease is quite significant, ranging from 8% to 35% of cases. 2 This is most common in conjunction with mitral valve disease but association with a...

BACKGROUND Moderate secondary tricuspid incompetence has variable natural history if left unattended during mitral valve surgery. Recent data suggest progression of the secondary tricuspid incompetence over time.Secondary moderate tricuspid regurgitation in rheumatic mitral valve disease may regress after mitral valve surgery without direct intervention. OBJECTIVES The present study was done ...
متن کاملRight atrial pressure-volume relationships in tricuspid regurgitation.
Pressure-volume relationships in the right atrium were examined before and after the creation of acute experimental tricuspid regurgitation in pigs. A 1.3 kHz multielectrode impedance catheter with a measuring current of 4 mA was used to determine instantaneous right atrial pressure and relative blood volume; right atrial dimension was assessed simultaneously with ultrasonic crystals attached t...
متن کاملAsymmetry of right ventricular enlargement in response to tricuspid regurgitation.
BACKGROUND Analysis of right ventricular adaptation to tricuspid regurgitation was studied in 10 heart transplant recipients following inadvertent endomyocardial biopsy disruption of the tricuspid apparatus. METHODS AND RESULTS Echocardiography demonstrated progressive diastolic right ventricular cavity enlargement (19.5+/-5.0 to 30.3+/-5.4 cm(2), P<0.0002), with disproportionate elongation a...
متن کاملMild mitral and tricuspid regurgitation secondary to pericardial constriction.
A 26 years female presented with pericardial effusion. On follow up she developed pericardial constriction with new appearance of mild mitral and tricuspid regurgitation without any other pathology. Fibrosis along posterior atrioventricular groove could be responsible for regurgitation.
